Services are provided in order to allow individuals to remain and live successfully in their own homes, function and participate in their communities. PAC services support & enable activities of daily living, self-care, mobility, prompting, reminders, supervision, and monitoring of needs to ensure health, safety, and welfare of the individual.
Community-Based Habilitation Services - Individual services are services provided outside of the individual's home that support learning and assistance in the areas of self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills. Community-based activities are intended to build relationships and natural supports.
Respite care services are provided to individuals unable to care for themselves. Respite care services are furnished on a short-term basis in order to provide temporary relief to those unpaid persons normally providing care. Respite can be provided in the individual's home or in a non-private residential setting, a caregiver's home, etc.
Residential Habilitation and Support (RHS) are hourly services provided up to a full day (24 hr basis) of services and/or supports that are designed to ensure the health, safety, and welfare of the participant and assist in the acquisition, improvement, and retention of skills necessary to support individuals to live successfully in their own homes. There are services which are: Level 1, Level 2 and Daily Rate
Behavioral Support Services means training, supervision, or assistance in appropriate expression of emotions and desires, compliance, assertiveness, acquisition of socially appropriate behaviors, and the reduction of inappropriate behaviors.
