Exciting Opportunity to Provide Nursing Case Management!
As our Agency Nurse, you will provide case management oversight of medical services to coordinate the healthcare needs of the individuals we serve:
- Coordinate and oversee activities associated with individuals’ healthcare needs
- Monitor medical services as outlined on physician’s orders
- Complete monthly assessments and reports for assigned individuals
- Facilitate communication and serve as a liaison between the agency, physicians and other external medical personnel to ensure consistency of medical information
- Provide ongoing education and training to direct support staff and clients, as needed including signs and symptoms of illness and symptoms of ongoing diagnoses, medication administration, etc.
- Assist the management team in implementation of medical quality assurance programs

Participant Assistance & Care (PAC)
Services are provided in order to allow individuals to remain and live successfully in their own homes, function and participate in their communities. PAC services support & enable activities of daily living, self-care, mobility, prompting, reminders, supervision, and monitoring of needs to ensure health, safety, and welfare of the individual.

Services are services provided outside of the individual's home that support learning and assistance in the areas of self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills. Community-based activities are intended to build relationships and natural supports.

Respite
Respite care services are provided to individuals unable to care for themselves. Respite care services are furnished on a short-term basis in order to provide temporary relief to those unpaid persons normally providing care. Respite can be provided in the individual's home or in a non-private residential setting, a caregiver's home, etc.
